Your own contact information, which can be viewed by enabled users (such as your physician) includes your home address, phone number and email address. Please remember to update this information if it happens to change.
Editing Your Contact Information
- Your own contact information, as well as your doctor's and any reference or emergency medical contacts, can be accessed by clicking the PERSONAL button on the side menu. Three subcategories should appear on the side menu.
- To edit your own contact information, click the CONTACT INFO subcategory button. Your contact information was added automatically when your account was set up. If it has changed, click Edit Personal Data.
- Edit any erroneous fields or fill in blank fields as you wish, remembering that fields with red stars (first name, last name, street address, city, state/province, and postal/zip code) MUST be filled in for the edit to be completed.
- Click save.
